This form is a confidential way to report an incident, concern, or experience that happened at AREA 506. Your report helps us better understand what’s happening in our space and identify ways we can improve safety and inclusivity.

All incidents will be addressed on a case-by-case basis in consultation with AREA 506 staff when appropriate.
